- Complete a current Impact Baseline Assessment if needed
Baseline Impact Test Assessment:
If your Impact test is NOT up to date you will receive an email from either James Blake, Brian Falencki, or Kristin Paisley with a private link to complete your baseline assessment virtually. All 7th, 9th, 11th-grade student-athletes are required to have a new baseline assessment completed for their 1st sports season. If a current 8th, 10th or 12th-grade student did not participate in a sport last school year, they must also have a new baseline assessment completed. These communications will go out after you have completed your registration. Please note: athletes will do a baseline concussion test whether their sport is deemed contact or non-contact.
If you have questions or concerns please contact James Blake, Brian Falencki, and Kristin Paisley at firstname.lastname@example.org or Amy Holland at email@example.com.
Thanks! Have a Great Summer!!
